About our location
The Rehabilitation and Nursing Center – Kentridge, formerly known as Rehabilitation and Nursing Center – Kalamazoo Avenue, is located on a 10 acre campus, our indoor and outdoor spaces are designed to provide opportunities for improving independences, strength, and confidence. Two main floor courtyards provide easy-to-access outdoor space for rest and renewal. Most patient rooms are private, and have an attached bathroom and shower, television with cable, phone, and a large window. Each unit has a large and accessible living and dining space, and a menu-based meal service is provided.
Inpatient subacute rehabilitation program
For over 30 years Corewell Health Rehabilitation and Nursing Center – Kentridge has provided a comprehensive inpatient rehabilitation program where a team of clinical professionals provide coordinated medical, nursing, clinical and therapy services for people who need to stay in the program 24 hours/day, until they are stronger and ready to return back to community based living. This 77 bed program is located on the first floor of a two story building, and is designed to meet the needs of people who would benefit from an integrated interdisciplinary approach to care, but do not meet the criteria for an ‘Acute Rehabilitation Program’, such as Corewell Health’s Inpatient Rehabilitation Center at Blodgett Hospital. “Subacute” rehabilitation programs are provided outside of the hospital, in skilled nursing facilities (SNF).
Program description
As specialists in rehabilitation, our program team members are passionately focused on helping every person who comes through our door to improve their overall function and abilities. The entire rehabilitation team works with the person served and their families or support systems to develop a treatment plan that addresses the goals and preferences of each person.
The program serves people with orthopedic and medical diagnoses, ortho-trauma, and neurologic conditions such as ischemic and hemorrhagic stroke, rehabilitation following brain tumor , hypoxic and anoxic brain injury and traumatic brain injury. The average length of stay for all people served is 21 days, however length of stay may be shorter or longer, depending on each person’s diagnosis and rate of progress. We are able to safely manage medically complex issues such as:
- Tracheotomy care and weaning
- Enteral feeding (‘tube feeding’)
- Wound care, including management of vac dressings
- IV medications
- TPN

A physician trained to assess disability and specialized in rehabilitation.
Care manager
I am a registered nurse who acts as a liaison to your providers and insurer.
Social worker
I help families put healthy habits in place, connect patients to mental health and other resources.
Recreational therapist
I create, coordinate, and execute recreational-based treatment programs.
Clinical dietitian
I design and assist with nutrition programs to improve the health of patients.
People served by the program:
- Are medically stable and no longer require acute hospitalization.
- Breathe independently (the program does not accept people on vents).
- Have the potential to participate in and benefit from daily skilled nursing and/or therapy services.
- Have a reasonable expectation of discharge to a community-based living environment, such as home or assisted living, or to a next step rehabilitation program.
- Do not demonstrate behavior that is a danger to self or others.
- Are age 16 or older. Younger persons may be served on a case by case basis.
Neuro rehabilitation
In addition to providing general medical rehabilitation, this location has CARF accredited stroke rehabilitation and brain injury rehabilitation programs. These specialty programs focus on the unique medical, physical, cognitive, and psychosocial needs of persons served following stroke or acquired brain injury. In order to maintain this specialty CARF accreditation level, the program must demonstrate that it has the commitment, capabilities and resources to assist each person served to optimize their recovery following neurologic injury. Staff receive training specific to brain injury and stroke, and many team members have advanced level certifications. Many team members have met the requirements for ‘Certified Brain Injury Specialists', a training program focused on brain injury rehabilitation that was developed by the Brain Injury Association of America.
In addition to the clinical services provided, the program also demonstrates a commitment to participating in ongoing research. Program team members have knowledge of community and state-wide resources, can assist with providing information about local support groups, and are involved in professional communities so that we continue to advance staff knowledge and expertise.
Onsite physical medicine and rehabilitation (physiatry) providers coordinate and make recommendations to the program medical and rehabilitation team to optimize outcomes. Clinical consulting neuropsychology and neuro-psychiatry services are available as well.
Emerging consciousness program
Our inpatient subacute rehabilitation program emerging consciousness program addresses the unique needs of people who have low levels of consciousness following a severe brain injury. The program includes interventions to promote awareness and arousal, reduce risk of medical and physical complications, and provides and adjusts stimuli in order to provide the best environment and opportunity for improvement.
Therapy services
Core therapy services provided include physical, occupational, and speech therapy. Therapy intensity is based on each persons’ diagnosis and tolerance, however most people will receive 1.5-3 hours of therapy a day. Therapy is provided five to seven days a week. In additional to these core therapies, the program offers recreational therapy and neurologic music therapy. Therapists have a variety of advanced training and/or certifications to ensure the best opportunity for recovery. Therapy is provided in a variety of locations, including two therapy gyms, private treatment rooms, a rehab kitchen, the rehabilitation dining area, and outdoors in our mobility pathway or indoor courtyards. The program also has a practice apartment, complete with a traditional bathtub, kitchen and double bed, which provides a great spot to practice newly acquired skills.
The program is able to work with a variety of insurances, including Medicare, Medicaid, many commercial insurances, Michigan Auto No Fault and Worker’s Compensation. Upon referral, our access team will identify and clarify coverage and benefits available.
CARF accreditation is voluntary, and provides a way for to open up the program service delivery and processes to outside scrutiny to improve the quality of rehabilitation provided. This program has a history of successful CARF accreditation surveys for over 30 years.
Accreditation is official recognition that this program is guided by internationally recognized service standards and best practices. The accreditation process consists of onsite in-depth reviews and consultations to help the program achieve the highest quality of care for the people we serve. The program is one of very few licensed skilled nursing facilities in the country that is accredited by CARF International as a “Comprehensive, Integrated Inpatient Rehabilitation Program”. This voluntary accreditation is an official recognition that the program is guided by internationally recognized standards. The program has been CARF Accredited for over 30 years, and has specialty accreditation in the areas of brain injury rehabilitation and stroke rehabilitation.
