Long-term care doesn’t simply mean nursing home care. Our facilities are designed to help residents restore and maintain the highest possible level of emotional and physical well-being. We pledge to preserve the dignity and quality of life for each and every resident we serve.

Looking for short-term rehabilitation?

Short-term inpatient rehabilitation, also known as subacute rehabilitation is goal-oriented treatment provided to those after a surgery, serious injury or illness. Subacute rehabilitation is time-limited with the express purpose of improving functioning and discharging home.

Time and again, research has shown that the healthier the surroundings, the healthier the person. That’s why our communities feature amenities designed to encourage social interaction and private time in a warm and caring environment. We work hard to help our residents stay mentally and physically active through a variety of services and daily activities.

Our residents range in age and diversity. Typically, residents choose long-term care when they require more medical attention than their family members can provide. As part of West Michigan’s largest health care system, our long-term care programs provide access to award-winning health care. We have rehab and nursing centers where our long-term care programs are located. Each of these facilities is designed to help residents live a healthy and active life to the greatest level possible. All of our locations offer a wide range of benefits including:

  • Continuity of care throughout the entire journey from the hospital to rehab
  • Medical provider coverage and therapy available up to seven days a week depending on patient needs
  • Private and semi-private rooms available
  • Beautiful, spacious and unique treatment areas for patient care such as therapy gyms, ADL kitchens and ADL apartments
  • Animal therapy
  • Garden spaces
  • Recreational therapy
  • Programming and recreational interventions seven days a week
  • All programs and interventions are person-centered and individualized according to a resident or patient’s preferences and skills
  • Many one-on-one recreational interventions provided for those who do not go to group programming
  • Physical activities, music groups, games, special meals, movies, support groups, inter generational programs, outings to zoos, stores, restaurants, sporting events, etc.
  • Beauty salon and barber services
